Investigation Summary
On November 20, 2003, Employee #1, a maintenance employee, pulled a refuse vehicle into the shop for scheduled brake adjustments. Employee #1 set the park brake, and got out of the truck. He placed a wheel chock behind the front driver's side wheel. Employee #1 was near the side of the truck at the rear of the vehicle. It is unclear what actually occurred, but from the investigation it appeared that the truck was left in reverse gear while the truck was running. It is assumed that Employee #1, for some reason, revved the engine from the back of the truck. The truck lunged backwards and went up over the wheel chock, causing the truck to turn, and pinned Employee #1 against a concrete pole on the outside of the garage door. The pressure of the truck pushing Employee #1 against the pole caused the pole to break. Employee #1 was found in the alleyway adjacent to the business meanwhile, the truck continued its travel out the garage door and came to rest against a vehicle in the street. When rescue teams arrived on the site the truck was still running. Employee #1 was killed.
