Investigation Summary
Employee #1 was working alone as part of a cleaning crew in the slaughter area of a meat processing plant. He was using a scrub brush on an extendable pole to clean the ceiling above the tail puller. He positioned himself so that he was straddling the hide pulling arm when he inadvertently tripped the switch attached to the overhead conveyor with the scrub brush. His left leg was on the portion of the machine that holds the animals head and his right leg was on a small sheet metal platform. The arm cycles in a 90-degree arc towards the back wall of the machine. At the top of the arc the arm tucks into a cavity in the back wall of the machine. The tail puller is designed to separate the cow hide from the carcass by inserting an arm, approximately 32 inches in length and running horizontally between the hide and the carcass. The arm then pulls in an up and away motion tearing the hide away from the carcass. When the machine cycled, Employee #1 couldn't get his leg out of the way and the arm moved quickly enough to pull his leg into the cavity. He sustained a compound fracture to the right femur as well as a laceration to his right foot.
