Investigation Summary
At approximately 9:25 a.m. on January 21, 2003, Employee #1 was part of a concrete stripping crew. As a beam was in the stripping process, the 60-foot long form rolled forward from its resting place on the forks of the fork lift. Employee #1 became pinned between the Cunningham beam form and the concrete deck support trusses. Employee #1 was hospitalized.
