Investigation Summary
At approximately 11:00 a.m. on November 30, 2002, Employee #1 was clearing a jam on a piece of machinery called an "unscrambler." Employee #1 waved to the operator to lock out the machine; however, the operator did not understand the signal. Employee #1 jumped onto the unscrambler. It started automatically trapping the Employee #1's leg, breaking it in several places before coworkers could extricate him. Employee #1 was hospitalized.
