Investigation Summary
Employee #1 was accessing a scaffold deck climbing the scaffold ladder. He was holding on to resin paper in one hand while gripping the scaffold ladder. He grabbed the top rung of the ladder, which was approximately 20 ft high, and his hand slipped. Employee #1 fell and was hospitalized with a fractured back.
