Employee Dies When Conveyor Fell On Him

Employee #1 was working with two coworkers loading a 10,000-pound conveyor rack system into the back of a 48-ft truck. Employee #1 was inside the truck while one of his coworkers was pushing the rack system from outside the truck using a forklift. The conveyor fell on top of Employee #1 while it was being pushed into the truck. Employee #1 was killed.
