Employee slips and falls in water and drowns

Employee #1 brought a 23 ft sailboat around to a floating dock at a marina to fill its tank with fuel. Employee #1 stepped off the vessel and slipped and hit his head on a metal dock cleat used to tie boats and he fell into the murky water. A coworker saw Employee #1 fall and approached the area immediately and called the emergency services while another coworker went for a diver. They recovered Employee #1's body after more than 15 minutes underwater. He was taken to the hospital where he was pronounced dead.
