Investigation Summary
On November 18, 2002, Employee #1, the owner of the company, and a coworker were in the process of initially placing a pier at the base of a completely excavated 50 year old poured concrete basement wall footing. A lifting plate and an approximately 3-foot long cylindrical steel piling were being position up under and against the base of the previously cracked section of the poured wall. As the 3-foot long piling cylinder was being pulled back by hand for initial placement, the wall section cracked the remainder of the way through the footing, causing the wall to fall and collapse onto Employee #1 while the coworker was able to escape the path of the falling wall. Employee #1 suffered fatal fractures caused by the collapsing wall.
