Investigation Summary
On November 13, 2002, Employee #1 was working with a large paper roll. The roll was on a lift table. As the roll returned from the backstop, it came off the lift table and crushed Employee #1 between the roll and another table, killing the employee. Investigation revealed that it is company policy to walk the rolls back after hitting the backstop to slow the rolls down.
