Investigation Summary
At approximately 8:45 a.m. on October 24, 2003, Employees #1, #2 and #3 were three members of a four man crew working to repair a 4-in. natural gas intra-field transfer line (900 psi). The line had developed a pinhole leak. The outer coating on the line had become damaged and had fallen off, allowing barnacles to develop on the exposed pipe surface. Employee #1 was the diver and Employees #2 and #3 remained on deck. Employee #1 was using a hatchet to remove the barnacles when the line ruptured, releasing the natural gas. The crew was using a gasoline powered air compressor, located on deck. As the natural gas escaped the water, it ignited resulting in an explosion. Employee #1 was killed while underwater by the explosion. Employees #2 and #3 were severely burned and were transported by helicopter to a local hospital and hospitalized.
