Employee Killed in Cave In

At approximately 1:00 p.m. on January 3, 2003, Employee #1 and a coworker entered a 20-ft-long by 5-ft-wide by 9.5-ft-deep trench to secure a 14-ft anchor pole to a rebar cage that had previously been lowered by a track hoe. The anchor pole served as an anchor point for a guy from a utility tower. Both employees along with two additional employees had previously added hardware to the utility tower thus requiring the addition of three more guys for support. The employees were attaching the anchor pole to the rebar cage with tie-wire when one whole length of wall toppled over onto both employees. Employee #1 was completely covered and the coworker was partially engulfing. The coworker suffered only minor bruises and scrapes and did not require hospitalization but Employee #1 died of suffocation.
