Investigation Summary
On December 5, 2006, Employee #1 was using a 24-ft extension ladder that was extended all the way, to access the roof to inspect a water leak. After checking the leak, Employee #1 was descending the ladder. Employee #1's daughter had just arrived, and he told her to hold the ladder. As the daughter approached the ladder, the ladder slipped down. Employee #1 lost his balance and fell. Employee #1 struck his head on the edge of the concrete walkway, sustaining fatal fracture injuries.
