Employee died while working on a truck

On January 9, 2003, Employee #1, a truck mechanic, and a coworker were attempting to repair an emergency parking brake on a truck. Employee #1 was located in an enclosed area near the trucks drive shaft access door and platen (the flat, horizontal oriented part of the compacting mechanism). Employee #1 instructed his coworker to start the ignition of the truck. For some reason, the compactor was in the automatic mode, half revolution, and once the air pressure built up in the air lines, the compactor completed its cycle. This sent the platen to its resting position, crushing Employee #1. Employee #1 was hospitalized and pronounced dead due to injuries sustained.
