Investigation Summary
On December 10, 2002, Employee #1 and a coworker were told to go up on the roof of the company warehouse and clean out the gutters. The coworker fell through a skylight on a cab of a pickup setting, inside the shop. He was not injured. Employee #1, who was working with the coworker on the roof, came down to see whether the coworker was hurt. Then, Employee #1 and the coworker were told to go back on the roof and patch the hole that was created when the coworker fell through the roof. Suddenly, Employee #1 fell through another skylight, which was adjacent to the one the coworker fell through. Employee #1 was killed.
