Investigation Summary
At approximately 2:00 a.m. on April 11, 2003, Employee #1 and a coworker prepared a tub by pumping product from a tank into the tub and adding maltrin. The tub was heated by steam and the product began to overflow. The tub then exploded, destroying the dryer area and killing Employee #1.
