Investigation Summary
Employee #1 and a coworker were working in a personnel platform (manbasket) that was elevated to a height of approximately 10.75 feet on the forks of a rough terrain forklift. The employees were hanging a banner on a pre-installed wire cable. The manbasket and forklift were positioned so that they were parallel to the banner. The coworker was standing in the south end of the manbasket and was clipping the banner to the cable. Employee #1 was standing in the center of the manbasket and was feeding the banner to the coworker. The manbasket was equipped with only one fork attachment sleeve. Employee #1 walked over to the side of the basket where the coworker was standing. When the weight of both employees was positioned on the end of the manbasket where the attachment sleeve was located, the other end of the basket, which was unsecured, began to tip upward. This threw the two employees out of the basket, and the basket continued to flip off of the forks. The basket fell onto Employee #1, killing him.
