Employee is crushed and killed by carpet wrapping machine

Employee #1, a mechanic, was inspecting the rollers of a perpetual wrapping machine when a coworker, the operator of the machine, pointed out a loose bolt on the upright. Employee #1 asked the operator to hold down the "ram in" button, which brings the ram in to insert the cardboard inserts into the ends of the carpet roll, while Employee #1 reached in and placed his upper body between the upright and the ram so that he could tighten the bolt. Just seconds after he started to tighten the bolt, the ram sprang back and pinned Employee #1 against the upright and he was crushed and killed. The operator holding the "ram in" button claimed that he did not release the button.
