Employee Is Killed in Fall from Scaffold

At approximately 10:30 a.m. on August 18, 2002, Employee #1 was painting overhead precast double-tee roof decking at a construction site in Salisbury, NC. He was standing on a fully-planked, mobile, tubular-framed scaffold, using a Speed-flo EXL-DC spray unit with a Quickreach telescoping spray pole that extended between 7.5 feet and 12.5 feet. Employee #1 was climbing off the scaffold when he slipped and fell approximately 13 feet, 8 inches to the concrete floor. He sustained massive head trauma and was transported to the hospital, where he was pronounced dead. There were no witnesses to the accident. The scaffold had standard railings on all sides.
