Investigation Summary
On October 20, 2003, Employee #1 was operating a Crown forklift, Model Number 30RC-TT, at a SAM's Club in Anchorage, AK. The employee attempted to decelerate the forklift by placing his foot outside the protective control space of the cab. As the forklift came to a stop, his foot became lodged between the forklift and the vertical support of a display rack, crushing his foot and ankle. He was hospitalized at Providence Hospital. The employer had a formal training program for forklift operators that involved interactive computer training and supervised training programs for operators prior to certification. Employee #1 was a forklift operator in training which involves a designated period of observed forklift operations prior to certification. The model being operated at the time of the accident required the employee to stand on a spring loaded platform with one foot while applying foot pressure to a foot pedal with the other foot. The operator stands in a curved, contoured continuous curvilinear metal shielding which protects the operator from outside impact. Employee #1 indicated that he extended his foot outside this protective space prior to impact with the metal support beam.
