Employee amputates finger after being crushed by fork

On September 5, 2003, Employee #1, an equipment operator, was squaring up a stack of jet pallets to be loaded on a B747 aircraft. While squaring up the pallets, the right fork slid along the ground and slid to the center of the rack, where it became dislodged. Two coworkers had replaced the fork on the rack when it jumped and crushed the employee's finger tips at the tip of the fork. Employee #1's fingers were crushed and about 0.25 in. of his middle and ring fingers on his right hand were amputated. Employee #1 was hospitalized.
