Investigation Summary
At approximately 12:45 p.m. on July 21, 2003, Employee #1 was attempting to walk down a sloped area covered with exposed peat rock and wet tundra grass. For some reason, he fell striking his right lower back on a form stake that was protruding approximately 3 inches to 4 inches from the ground. The stake was acting as a brace to hold up a 2-inch by 4-inch by 12-foot form and was protected with a cap. Employee #1 was hospitalized and treated for contusions and abrasions to his right side.
