Investigation Summary
At approximately 9:15 a.m. on December 19, 2002, Employee #1 was helping another employee mount a mailbox adjacent to the southbound lane of U.S. Route 1. While securing the box to the metal posts with his back to traffic, Employee #1 was struck in the back of his head by a piece of 3-inch-diameter pipe that had become dislodged and was overhanging a flatbed truck, as the truck passed by. Employee #1 died later that evening as a result of his head injury.
