Investigation Summary
On March 3, 2003, an employee was working on the flat roof of a commercial building. He was unrolling and installing a 40-ft by 100-ft sheet of Kelly 2001 Roofing Rubber. There were approximately 43 skylights on the roof, none of which had guardrails or an adequate cover to support the weight of a person. The covering on the skylights was a thin plastic dome that allowed light through. The employee fell through one of the skylights and landed on the ground, approximately 45 feet, and died.
