Employee Killed When Lift Basket Falls

On February 7, 2003, a high-reach fork lift truck was being used to raise a job-made personal lift basket so that another employee could replace a burned out light tube at approximately 40 feet above floor level. The operator and Employee #1 had secured the basket to the forks, but it was improperly secured, causing the basket and Employee #1 to fall to the concrete floor below. He died from fatal head trauma.
