Employee lacerates hand in roller machine

On November 12, 2002, Employee #1 was inserting paper into a dryer roller. A 12-in diameter rope drive pulley fell down and hit the roller. Employee #1 was hospitalized with lacerations to his first, middle, and ring fingertips on his right hand. Investigation revealed that the set screws on the pulley wore out, causing the pulley to drop down and strike the roller.
