Investigation Summary
A superintendent used a rough terrain forklift to pick up a wooden box containing sixteen windows. He did not secure the box to the forks. He elevated the box up to each floor of a building where Employee #1 and a coworker unloaded them. On the third floor, Employee #1 climbed into the box and handed windows to the coworker. The forklift operator then saw the box tilt to the right and it, along with Employee #1, fell to the ground. Employee #1 was hospitalized with fractured back and ribs as well as injuries to his ankles.
