Investigation Summary
On July 30, 2002, Employee #1 was on the second floor of a single-family residence under construction. Employee #1 was using a stepladder to install 8-in. HVAC flex duct in the rafters. Employee #1 was descending the ladder when he lost his balance and fell through an unguarded floor opening that was less than 18 in. from the base of the ladder. Employee #1 fell 18-ft 3-in. through another opening in the first floor and landed in the basement. Employee #1 sustained injuries to his head, neck, and spine, and was hospitalized for two days.
