Investigation Summary
Employee #1 and a coworker were running a test stroke on a 300-ton part revolution mechanical power press after maintenance repairs. The press was being utilized in a single stroke hand-fed application to manufacture an automotive reinforcement body side outer panel. Upon actuation by the press operator the press cycled once and as Employee #1 reached for the materials in the die the press unintentionally cycled again. The press die crushed across both Employee #1's palms amputating eight fingers but leaving both thumbs. Control circuits were not installed with control redundancy safety features to prevent an unintended stroke. Hazardous energy control procedures were not utilized prior too or during the maintenance. Hazardous energy control procedures did not address additional requirements for testing when the maintenance was completed. The test strokes could have been completed from a console a safe distance from press operations without materials in the press for the hand fed application. Employee #1 suffered the amputation of eight fingers and was hospitalized.
