Investigation Summary
On June 14, 2002, Employee #1 was cutting Styrofoam sheets on an electric Styrofoam- cutting machine, and he contacted one of the energized cutting wires with his face. He was wearing short pants and his legs were in contact with the steel frame of the machine, providing a good electrical ground that allowed the current to flow through his body. Employee #1 was electrocuted. Employee #1 was found face-down, slumped over the machine's frame, by a coworker.
