Employee Struck in The Head and killed by A Rig Handle

Employee #1 and his coworkers were using a forklift truck to place a lifting rig around a septic tank. The forks of the forklift truck were in the in openings of the handle of the lifting rig. The septic tank was inside the mold. The septic tank was placed on the ground. Employee #1 removed and replaced the pins, which enabled the fork truck operator to raise the rig handle. Employee #1 removed two anti-spinning pins. The tank was raised with the fork truck and the rig and the tank spins so the bottom was at the bottom. After removing the anti-spinning pins Employee #1 was supposed to stay to the side of the fork truck operator to remain in his view. For some reason after removing the anti-spinning pins, Employee#1 walked around the back of the septic tank, when the rig handle fell and hit him on the head, killing him. The forks of the forklift truck were supposed to extend 12 ft to 18 ft past the rig handle. The forks of the truck were supposed to apply an outward pressure to lock the forks in place in the rig.
