Employee Dies after Being Struck by Forklift Counter-Weight

At approximately 2:23 p.m. on April 29, 2005, Employee #1 with Liftach Equipment Companies, Inc., was struck by a forklift counter-weight that fell on his upper left leg. The counter-weight had been previously removed from the forklift. Employee #1 was transported to a hospital, where he died of his injuries later that day.
