Investigation Summary
On March 31, 2003, Employee #1 moved a scissor lift from the fueling area to the maintenance area. He then operated the machinery manually by walking with the controls backwards. Somehow, he died when he became pinned between the machine and a backhoe that was already parked in the maintenance area.
