Investigation Summary
At about 11:50 a.m. on June 17, 2005, Employee #1 was working in the warehouse. He was asked to lower a pallet of clothing. He drove a forklift to the location, where the box was stored. Employee #1 picked up the pallet and set it down on the floor. While lowering the pallet, a box from another pallet fell down. Employee #1 decided to put the fallen box back on the pallet on an about 10-ft high rack. He asked a coworker to operate the forklift and lift him up. Employee #1 stood on one of the forks of the lift, while holding the box. As the forks reached the desired height, Employee #1 shouted "stop," but a few seconds before that, the tip of the forks hit edge of the pallet and caused the forks to move. The movement caused Employee #1 to loose his balance, and he fell to the floor. He sustained a severe head trauma and died upon arrival at the hospital.
