Employee Injures Arm and Shoulder While Operating Forklift

On April 6, 2005, Employee #1, a framing foreman, was using a Gradall Reachlift industrial truck, Model Number 534D, to stock lumber at a housing tract with several-acres under construction. The base around one building, still under construction, had not yet been backfilled, creating a 3 foot trench. As Employee #1 was driving around the corner of the building, the truck slid into the open trench. Employee #1's left arm became caught between the building and the truck, fracturing his arm and injuring his shoulder. Employee #1 was taken to a nearby hospital, where he was hospitalized and treated for his injuries.
