Investigation Summary
At approximately 7:30 a.m. on March 14, 2005, Employee #1 was using a New Holland Ford 655-E front end loader, Model Number 699000001, Serial Number 031018192, to backfill gravel into a 2-foot deep trench in front of a 6.5-foot-high CMU retaining wall. He drove the front end loader with a bucket full of gravel down an incline and parked the loader so that the bucket was at the end of the trench. He was then instructed by a coworker to help scoop the gravel from the bucket into the trench. After unloading the gravel from the bucket, Employee #1 bent over to lay landscaping fabric over the gravel. At this point, the front end loader moved down the incline and pinned him against the retaining wall. He was crushed and killed.
