Investigation Summary
On August 5, 2004, Employee #1, a carpenter/laborer, was working for a residential general contractor in the extensive remodeling of an existing single family residence. Employee #1 was installing plywood sheathing on the roof of the residence, putting on the first course of plywood above the starter boards. The roof had a slope of 6 to 12 and the eave of the roof was 17 feet, 1 inch from the ground. A 2-inch by 4-inch board had been nailed between two rafters and sheets of plywood were stacked behind it; Employee #1 pulled from that stack as needed. To perform his work, Employee #1 reached for and hoisted a sheet of plywood, while kneeling on one knee and bracing himself with his other foot on one end of the 2-inch by 4-inch board. While doing so, the end of the board on which his foot rested suddenly became detached from the rafter. Employee #1 lost his balance and fell off the eave of the roof, glancing off a masonry wall and landed in the neighbor's back yard. He was hospitalized and treated for two broken ribs, and two herniated disks. During the accident investigation, it was revealed that Employee #1 was not protected from falling by scaffolding, guardrails, personal fall protection systems, or any other means of fall protection.
