Investigation Summary
At approximately 1:00 p.m. on September 23, 2004, Employee #1, a general laborer, was working for a home-remodeling contractor. He was operating a Ryobi 10-in. table saw, Model Number BTS, Serial Number XX032251994. The saw struck him and he suffered an amputation of his right index finger and severe lacerations to his other fingers. Employee #1 was hospitalized were his nearly-amputated finger was reattached and his other fingers sutured.
