Investigation Summary
At approximately 10:30 a.m. on July 27, 2004, Employee #1, a cable TV installer, was climbing a pole during a training exercise. He was tied off to the pole when the pole broke off at the bottom due to dry rot. The pole and Employee #1 fell. Employee #1 was hospitalized for multiple compound fractures of his left arm.
