Investigation Summary

At approximately 9:30 a.m. on May 6, 2004, Employee #1 working for Little Company of Mary Hospital, approached a coworker to discuss billing data. The coworker was standing at a PC terminal along the right side of a 42-in.- wide hall leading to patient examination rooms. The PC table extended into the aisle, as did a waste paper can and draftsman's chair, constricting the aisle to 32-in. wide. Employee #1 thought she could squeeze past the coworker and then turn and face her. As Employee #1 passed behind the coworker, she intended to push the station's drafting chair out of her way with her foot. Instead, she tripped on something, lost her balance, sliding down the left-hand wall and landing with her left ankle snapped under her. The coworker noted the fracture was severe and called for paramedics. Employee #1 was hospitalized at Little Company of Mary Hospital and had surgery to repair the compound fracture of the left ankle the next day.
