Investigation Summary
At approximately 1:35 a.m. on October 14, 2003, Employee #1, a truck driver, secured some "dunnage" or damaged corrugated boards to put in his trailer bed. He picked it up from the production area and was walking along the edge of the main aisleway, towards an unsecured trailer bed in Loading Door Number 6. He had carried the stack on his left shoulder while walking across several loading bays. In the meantime, a coworker picked up two pallets of cardboard and was travelling along the aisle between a row of stock rolls and a row of finished goods. The coworker was in the process of loading the trailer in Loading Bay Number 4. The equipment involved was a sit down LPG forklift commonly identified as Truck Number 9. His intention was to drive backwards all the way into the dock floor situated next to Bay Number 4 and make a right turn to unload the pallets. The coworker hit Employee #1 while driving backwards towards Loading Dock Number 4. Employee #1 fell as the truck hit him and its right rear wheel went over his left foot. When Employee #1 screamed, the coworker stopped the truck, put it in drive mode and drove forward, going over Employee #1's foot again. Employee #1 suffered a fractured left foot, but was hospitalized after the sixth day when he developed a condition called "phlebitis." Employee no. 1 stayed there for six days.
