Investigation Summary
On March 14, 2003, Employee #1, a security guard, was making his hourly tour of a truck trailer storage yard and was at the rear of the yard. He heard honking, indicating that a driver wanted the gate opened. Employee #1 began walking toward the gate. Another yard goat driver, a coworker of the warehousing and shipping company, observed the guard and offered him a ride up to the gate. Since the yard goat had no provision for safely transporting passengers, Employee #1 rode outside the vehicle's cab, between the cab and the fifth wheel. This area had a walking surface since entry to the cab was from the rear. During the ride up to the gate, Employee #1 fell off the vehicle and was struck by the driver-side rear wheels. He was hospitalized for 10 days with a fractured pelvis, a hematoma on his right leg, a gash on the back of his head, and an abdominal wound.
