Investigation Summary
At approximately 5:00 p.m. on March 4, 2003, a temporary employee was working as a machine operator under the direction and control of a health products packaging company. He was operating the IWKA packaging machine, Type CPR and Serial Number 375. The machine took filled and labeled bottles of product and placed them in cartons; embossed the cartons with the lot numbers and expiration dates; applied glue to the carton tabs; and sealed the carton. The embossing process was not working properly so the machine operator decided that the chain drive for the embossing wheel needed oiling. Although he has been a machine operator for two and a half year, oiling the embossing wheel is not part of his normal duties. This is done by a line mechanic. The employee obtained an Allen wrench from the line mechanic and removed the cover that was guarding the embossing wheel drive sprocket and the idler sprocket. He had not deenergized or stopped the machine or the embossing wheel drive chain. While oiling the chain, the glove on his right hand got caught by the chain and sprocket drive. Half of his middle finger on his right hand was amputated, but he was not hospitalized.
