Investigation Summary
At approximately 3:30 p.m. on October 8, 2002, Employee #1, a general laborer working for an employer engaged in the wholesale distribution of imported table linens, was throwing a box into a trash bin located in one of the docks when he slipped and fell. He fell 30 inches onto the trash bin and landed with one leg on the inside of the bin and the other on the outside of the bin. He was diagnosed with contusion in the scrotum and gross hematuria and was hospitalized. The employee was a contract employee.
