Investigation Summary
At approximately 2:45 a.m. on September 12, 2002, Employee #1, of R and J Askew Poultry Service, was part of a 12-man crew that was gathering chickens at a contract farm. He was walking in an open area between two poultry barns when he was struck by a powered industrial truck transporting caged poultry. Employee #1 was killed.
