Investigation Summary
At approximately 8:15 p.m. on April 02, 2002, Employee #1, a buffet kitchen worker at a hotel, went into a walk-in freezer to retrieve some meatballs. Soon after entering the freezer, Employee #1 slipped on a large patch of ice, which had accumulated on the floor, injuring his left knee. A coworker and hotel security provided assistance and Employee #1 was taken to a local hospital and diagnosed with a meniscus tear to his left knee. Despite being released from the hospital, it is likely Employee #1 will suffer permanent disability.
