Investigation Summary
On July 10, 2002, Employee #1 was using a 33-ft aluminum pole, cleaning windows approximately three stories above. Two of Employee #1's coworkers and his supervisor were cleaning a second story window. The coworkers and the supervisor were trailing and were occupied with one another. One of Employee #1's coworkers reminded him of overhead power lines that were between 32 ft and 30-ft-7-in. above. As one of Employee #1's coworkers turned to the other coworker to finish cleaning the second story windows, he saw Employee #1 retreating for the building. The pole that Employee #1 was using came in contact with four 16-ft power lines (23 KV phase to phase). The power lines were obstructed by a 20-ft tree that was underneath the lines. Employee #1's coworkers heard a yell and a zap. Both coworkers saw Employee #1 laying on the side of road towards the direction his retreat.
