One employee killed and two injured while unloading marble

Employees #1, #2, and #3 along with two other employees were unloading a trailer of stone products. They successfully unloaded several small containers of stone. At the back of the trailer, three crates of stone remained. Each crate contained 10 granite slabs measuring approximately 6 feet by 9 feet by 0.875 inches with the slabs set on the 9-foot-long side. Each slab weighed between 600 and 700 pounds. Two crates were on the left side of the trailer and one was on the right side. The workers decided that they could not move the crates with the forklifts they had available so they decided to move one slab at a time manually from the back of the trailer to the front where they could then pick them up with the forklift and remove them from the truck. They started with the box on the right side of the trailer. They successfully moved two slabs but when they tried to move the third slab, it broke and the remaining 6 slabs fell over onto the employees. Two of the employees were knocked out of the way and Employees #1, #2, and #3 were pinned by the slabs against the two crates on the left side of the trailer. Employee #1 was killed and Employees#2 and #3 were hospitalized with injuries.
