Investigation Summary
On May 28, 2002, Employee #1 was part of a roofing crew and had torn off the existing covering on a narrow, flat section of the roof. They were installing a new built-up covering. The new multi-layer covering called for the crew to first mop on hot tar and apply a layer of felt, then repeat the process two more times. The crew was applying the third layer of hot tar when Employee #1 sustained second- and third-degree burns to over 22 percent of his body. Treatment of his injuries would require multiple skin grafts and extended hospital and rehabilitative care.
