Investigation Summary
On April 13, 2002, Employee #1 was punching out hamburger patties on the Hollymatic Model 54 "Super" Patty Machine. The butcher had not shown up for the shift, and other employees were busy, so Employee #1, the shift manager, decided to produce the hamburger patties for the day. After attempting for several minutes to close the paper-dispensing unit, which attached to the patty-forming machine, she decided to begin operating of the machinery with the partially opened unit. However, as the patties were punched out, instead of falling into a straight line on the table's surface, they were deflected by the unclosed paper-dispensing system. Employee #1 compensated for the displaced paper-dispensing system, by placing her hand above the unit to catch the patties. As she began catching the patties, the knockout cup and the mold plate caught her plastic glove. The rotating parts pulled her hand into the machine and amputated her right index finger at the first joint. The machine was unguarded. She was transported to a medical center. At the hospital, medical personnel treated her injuries, and then released her.
