Employee caught between truck and retaining wall

An employee was working as a truck driver. The truck's parking brake was out of adjustment. Other mechanical problems were noted with the truck, also, by a UDOT inspector. The brake was designed to be adjusted by the operator of the truck. However, the driver had not been trained on the workings of this type of braking system. An accident occurred when the driver was caught between the truck and a retaining wall. He was hospitalized for his injuries.
